Project Transition
For 25 years, our innovative apartment-based programs in southeast Pennsylvania have provided hope and help to adults (ages 18+) with persistent psychiatric problems, including personality disorders and co-occurring substance abuse. We’re the recipient of the national Lilly Reintegration Award.
What the members of Project Transition share in common:

- Episodes of high acuity have left the person and family members exhausted.

- An open, supportive, normalized setting. Our programs are based in attractive apartment complexes that seamlessly blend into suburban communities.
- Highly integrated services. Comprehensive, on-site activities and supportive supervision build vital competencies: self-acceptance, healthy relationships, independent living skills, recovery self-management, educational and vocational gains, medication management, and good health.
- Multiple impact treatment and skill development. Our approach includes therapies, an emphasis upon belonging to a community, the careful use of medications, a wide range of rehabilitative and dialectical skill workshops, wellness and exercise activities, stress reduction practices, fun, and more.
- Substance abuse services. Experienced addiction counselors address the challenges of unified recovery from substance abuse and psychiatric difficulties.
- Levels of care & sustaining relationships. A graduated path fosters increased separation from the program without the loss of valued peer and staff relationships.
- 24/7 support. A responsive crisis response system provides peace of mind. Our top clinicians are always on call.
- The staff. Seasoned, licensed professionals provide on-site therapies, psych rehab, med management, and addictions treatment. Most of our therapists and physicians have 20+ years of experience; all of our psychologists are doctorates.
- Family friendly. Ongoing dialog between staff and family members is augmented by support meetings and quarterly educational seminars.
